Final Forms Explanation

Arcadia Local Schools Families,

This is a new process for our school district.   Your attention to the information below is required to make beginning of year information EASIER for all. 

Arcadia Schools has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign all annual enrollment and athletic participation forms for your students online. 

FinalForms saves data from year-to-year and season-to-season, meaning that you will never need to enter the same information twice! (Only if you have a change in address, phone, etc)

If we have your email on file, you should have received an email from FinalForms instructing you to set up your parent account.  Then you will able to view the information that we currently have on your student(s) and make changes as necessary.  You will not be receiving the packet of forms at the beginning of the year as you normally do.  This takes the place of those!

If you don’t receive an email from FinalForms, the district may need to update your email. You can go to the school website’s main page and click on the link to complete the form or email your new email address to Mr. McKee @ [email protected] along with your students name(s).

FinalForms also pre-populates information wherever possible, for each of your students, saving you time. Once you have completed your registration online, you will simply be asked to review and update your information at the beginning of each school year to verify the information is current and sign the forms.  You can also change information (address, phone, emergency medical, etc.) as it happens.

Your student(s), grades 7-12, will also be emailed (to their school email account) instructions to register for FinalForms and then can sign their sections of the forms as well.

Only ONE parent/guardian will need to register and use FinalForms. We will be communicating through your email for anything outstanding as well as special announcements. Please register at –  now and electronically complete ALL forms! 


For grades 7-12:  If your student is in or will be in a sport this year, please indicate that on that students account.  All sports forms will then be available for you to complete. The actual sports physical paperwork should be handed in to the Athletic Director.

The Arcadia School Board and administration are excited to share that we are beginning the process of writing a strategic plan for Arcadia Local Schools.  A strategic plan will allow our community to come up with both long and short term goals and plans for the district.  This planning process will take time and a great deal of input from the school community.  One way to start this process is to have students, staff, and parents/guardians complete surveys.  The feedback/data gathered in these surveys will help to guide and direct the decision making process moving forward. Surveys must be completed by Friday, June 4, 2021.
